For the most part, the index.php files in the public_html folders were replaced or just had infected index files added to them.  Swapping out the infected for backups (or deleting additional ones) brought my site back.

 

Tom created a zip with the index files that's linked to in a post on Yakezie.

Pretty much this. And they added index.php to almost every single directory. So you should which to keep and which to delete (back ups should give you this information).
